12 歲小孩現在真的在動椎間盤手術嗎?
這是那種會讓每個爸媽驚恐地一把抓起孩子書包的標題:小到 12 歲的孩子為了椎間盤突出上手術台。這是真的——而恐慌所歸咎的原因幾乎全都是錯的。這裡是誠實的故事。
這是一個給憂心忡忡的群組聊天室的完美標題:小到十二歲的孩子正在動脊椎椎間盤手術——是手機害的、是書包害的、是駝背害的。 於是每個爸媽都掂了掂孩子的書包,倒抽一口涼氣。
所以我們做了我們一向會做的事,跑去翻研究。轉折很精彩:這個嚇人的標題是真的——而恐慌釘在它頭上的每一個原因,幾乎全都是錯的。

對,這是真的——而對,它真的非常罕見
我們別把真的那部分講軟。孩子確實偶爾真的需要椎間盤手術,而且它真的往下觸及到 12 歲。在某家大型兒童醫院長達 19 年的系列個案裡,最年輕的患者就是 12 歲(McAvoy et al., 2019)。12 歲,有紀錄在案。
現在講講標題很方便地丟掉的那些脈絡。一篇回顧了全世界關於 15 歲以下孩子椎間盤手術醫學文獻的研究發現,橫跨 28 篇研究,有史以來總共只有 69 個案例——中位數年齡 13 歲(Loubeyre et al., 2024)。就算在一個高手術量的脊椎中心,青少年也只佔全部椎間盤手術的 2.6%(Wang et al., 2013)。而在 10 歲以下,它基本上不會發生:掃描 103 個幼童,你會找到零個突出(Urrutia et al., 2016)。就算突出真的出現在青少年的 MRI 上——大約 5.8% 的掃描——大多數也是偶然發現,沒有造成症狀,也不需要手術(Wan et al., 2023)。
所以這個標題誠實的翻譯是:真實、有紀錄,而且罕見得跟童年會發生的事一樣稀有。 不是流行病。幾乎可以肯定,不會是你的孩子。
真正的成因是什麼(爆雷:不是書包)
接下來就是恐慌真正崩解的地方。當你細看到底是什麼在驅動年輕人的椎間盤突出,兇手是遺傳與家族史、直接外傷、運動的沉重反覆負荷,以及較高的體重(Qi et al., 2023)。在那個大型醫院的系列個案裡,71% 是競技運動員(McAvoy et al., 2019)——他們是體操選手、舉重選手和橄欖球員,把一副還在生長的脊椎狠狠加載,不是被「坐姿不良」毀掉的孩子。
年輕的椎間盤,並不會像海報暗示的那樣,因為駝背而溫溫地「磨損」;它們往往是因為一次特定的受傷或一個遺傳體質才出問題。
那書包呢?規模最大的那篇回顧——69 篇研究、超過 72,000 名孩子——的結論是:「沒有令人信服的證據顯示書包使用的各個面向會增加背痛風險」(Yamato et al., 2018)。不是重量,也不是怎麼背。而那還只是針對一般的背痛;至於「書包造成椎間盤突出」這個念頭,證據更是薄弱。唯一一個站得住半隻腳的生活型態因素是長時間坐著——一篇單一的回顧性研究點出每天坐超過六小時——但就連那個測的也是坐著,不是螢幕或書包,而且它跟那套自信滿滿的「手機正在折斷孩子脊椎」的敘事,差了十萬八千里(Qi et al., 2023)。
「但它正在上升啊!」——真的嗎?
你會看到有人聲稱,青少年椎間盤突出正逐年攀升。但我們找不到能佐證這說法的資料。這個說法在綜述文章裡被當成背景一再重複,背後卻沒有任何紮實的族群趨勢研究撐著。真正改變的是,我們現在掃描得比以前隨性多了,因此發現了更多。比起「大爆發」,「一直很罕見,只是偵測得更好」是更公道的總結。
讓人安心的結尾
如果那件罕見的事真的發生了,預後是好的。匯總的資料顯示,手術後腿痛和失能戲劇性地改善,短期內同節段的再手術率約 1%(Than et al., 2024)——而且關鍵是,保守治療會先試,而且通常有效;手術是那條路失敗之後才動用的最後手段(McAvoy et al., 2019)。我們會保留一個誠實的但書:在數十年的尺度上,可能會有一小群、但值得注意的比例需要再動一次手術(Papagelopoulos et al., 1998)。不過「罕見、可治療,而且通常不用手術就能處理」,才是這件事真正的樣貌。
什麼時候才真的該看醫生
如果一個孩子或青少年有背痛或腿痛,並伴隨麻木、刺麻或無力,或是會把他們痛醒、或跟在一次真實受傷之後的疼痛,又或者大小便控制有任何變化,請認真看待。那值得一次正經的評估——不是一篇部落格的安慰,也不是一個恐慌下買的小工具。
誠實的結論
12 歲小孩動椎間盤手術是真實、有紀錄,而且極其罕見的——它是由基因、外傷和沉重的運動負荷所驅動,不是書包、手機或普通的駝背。以最好的證據來看,書包恐慌是一個迷思。所以,讓群組聊天室的驚恐退場吧,留意那些真正的紅旗徵兆,也別為了預防一個姿勢小工具根本預防不了的東西,去給你的孩子買一個姿勢小工具。
