12歳の子が本当に椎間板の手術を受けているの?
どの親も思わず子どものランドセルをつかんでぞっとする、あの手の見出し——12歳という幼い子どもが椎間板ヘルニアでメスを入れられている。それは本当です——そして、そのパニックが原因だと責めるものは、ほとんどすべて間違っています。正直な話をどうぞ。
不安いっぱいのグループチャットにはうってつけの見出しです。12歳という幼い子どもが脊椎の椎間板手術を受けている——スマホのせいだ、リュックのせいだ、猫背のせいだ。さあ、どの親も子どもの通学かばんを持ち上げ、その重さに息をのむ。
そこで、いつものように研究を読みに行きました。どんでん返しは良いやつです。この怖い見出しは本当。ただ、そのパニックがなすりつける原因のほとんどは間違いなのです。

はい、これは実在します——そして、はい、まぎれもなくまれです
本当の部分を、やわらげずにいきましょう。子どもが椎間板手術を必要とすることは、たしかにときどきあり、そして本当に12歳まで届きます。ある大きな小児病院の19年にわたる症例集では、最年少の患者は12歳でした(McAvoy et al., 2019)。12歳は記録に残っています。
この見出しが都合よく省いている前提も見ておきましょう。全世界の、15歳以下の子どもの椎間板手術に関する医学文献をレビューしたところ、見つかったのは28件の研究にまたがって、これまでにたった69例——年齢の中央値は13歳でした(Loubeyre et al., 2024)。症例数の多い脊椎センターですら、十代は全椎間板手術のわずか2.6%にすぎませんでした(Wang et al., 2013)。そして10歳未満では、それは本質的に起こりません。幼い子ども103人をスキャンしても、ヘルニアはゼロです(Urrutia et al., 2016)。十代のMRIにヘルニアが写ったときでさえ(スキャンのおよそ5.8%)、そのほとんどは偶発的なもので、症状を起こさず、手術も要りません(Wan et al., 2023)。

実際の原因は何か(ネタバレ:リュックではない)
ここでパニックは本当に崩れ落ちます。若い人の椎間板ヘルニアを何が引き起こすのかを見ると、犯人は遺伝と家族歴、直接の外傷、重いスポーツの反復負荷、そして高い体重です(Qi et al., 2023)。あの大きな病院の症例集では、71%が競技アスリートでした(McAvoy et al., 2019)。体操選手、重量挙げ選手、ラグビー選手が、成長中の脊椎を激しく酷使しているのであって、座り方が悪くて壊れた子どもではありません。
若い椎間板は、ポスターがほのめかすように猫背でおだやかに「すり減る」わけではありません。特定のけがや遺伝的な素因から、壊れる傾向があるのです。
そしてリュックは? 単独で最大のレビュー(69件の研究、7万2千人超の子ども)は、「通学かばんの使い方の側面が腰痛のリスクを高めるという説得力のある証拠はない」と結論しました(Yamato et al., 2018)。重さでもなく、背負い方でもなく。しかもそれは、ふつうの腰痛の話です。リュックが椎間板ヘルニアを引き起こすという考えは、さらに弱い。何らかの足がかりを持つ唯一のライフスタイル要因は長時間の座位です。ある後ろ向き研究が、1日6時間を超える座位を指摘しました。でもそれすら、スマホやかばんではなく座っている時間を測ったのであって、「スマホが子どもの背骨をへし折っている」という自信満々の物語とは、まるで別世界です(Qi et al., 2023)。
「でも増えているんでしょ!」——本当に?
十代の椎間板ヘルニアが年々増加している、と述べられているのを目にするでしょう。正直に言えば、それを裏づけるデータは見つけられませんでした。この主張はレビュー論文で背景としてくり返されますが、その後ろに堅実な人口動向の研究はありません。まぎれもなく変わったのは、いまや私たちがはるかに気軽にスキャンし、より多くを見つけるようになったことです。「昔からまれ、検出がうまくなっただけ」のほうが、「爆発している」よりも公平な要約です。
安心できる結末
そのまれなことが実際に起きても、見通しは良好です。まとめられたデータは、手術後に脚の痛みと障害が劇的に改善し、同じ高位での再手術は短期でおよそ1%であることを示しています(Than et al., 2024)。そして何より、保存療法がまず試され、たいていうまくいきます。手術はそれが失敗したあとの最後の手段です(McAvoy et al., 2019)。正直な但し書きを一つ残しておきます。数十年のあいだには、無視できない少数がさらなる手術を必要とするかもしれません(Papagelopoulos et al., 1998)。それでも、「まれで、治療でき、たいてい手術なしで対処できる」というのが、これの本当の輪郭です。
実際に医者にかかるべきとき
子どもや十代が、しびれ・ピリピリ・脱力をともなう腰や脚の痛み、眠りを妨げる痛みや本当のけがのあとの痛み、あるいは膀胱・排便のコントロールに何らかの変化があるなら、真剣に受け止めてください。それはきちんとした評価に値します。ブログからの安心でも、パニックで買ったガジェットでもありません。
