12 岁小孩现在真的在动椎间盘手术吗?
这是那种会让每个爸妈惊恐地一把抓起孩子书包的标题:小到 12 岁的孩子为了椎间盘突出上手术台。这是真的——而恐慌所归咎的原因几乎全都是错的。这里是诚实的故事。
这是一个给忧心忡忡的群聊的完美标题:小到十二岁的孩子正在动脊椎椎间盘手术——是手机害的、是书包害的、是驼背害的。 于是每个爸妈都掂了掂孩子的书包,倒抽一口凉气。
所以我们做了我们一向会做的事,跑去翻研究。转折很精彩:这个吓人的标题是真的——而恐慌钉在它头上的每一个原因,几乎全都是错的。

对,这是真的——而对,它真的非常罕见
我们别把真的那部分讲软。孩子确实偶尔真的需要椎间盘手术,而且它真的往下触及到 12 岁。在某家大型儿童医院长达 19 年的系列病例里,最年轻的患者就是 12 岁(McAvoy et al., 2019)。12 岁,有记录在案。
现在讲讲标题很方便地丢掉的那些脉络。一篇回顾了全世界关于 15 岁以下孩子椎间盘手术医学文献的研究发现,横跨 28 篇研究,有史以来总共只有 69 个案例,中位数年龄 13 岁(Loubeyre et al., 2024)。就算在一个高手术量的脊椎中心,青少年也只占全部椎间盘手术的 2.6%(Wang et al., 2013)。而在 10 岁以下,它基本上不会发生:扫描 103 个幼童,你会找到零个突出(Urrutia et al., 2016)。就算突出真的出现在青少年的 MRI 上——大约 5.8% 的扫描——大多数也是偶然发现,没有造成症状,也不需要手术(Wan et al., 2023)。
所以这个标题诚实的翻译是:真实、有记录,而且罕见得跟童年会发生的事一样稀有。 不是流行病。几乎可以肯定,不会是你的孩子。
真正的成因是什么(剧透:不是书包)
接下来就是恐慌真正崩解的地方。当你细看到底是什么在驱动年轻人的椎间盘突出,凶手是遗传与家族史、直接外伤、运动的沉重反复负荷,以及较高的体重(Qi et al., 2023)。在那个大型医院的系列病例里,71% 是竞技运动员(McAvoy et al., 2019)。他们是体操选手、举重选手和橄榄球员,把一副还在生长的脊椎狠狠加载,不是被“坐姿不良”毁掉的孩子。
年轻的椎间盘,并不会像海报暗示的那样,因为驼背而慢慢地“磨损”;它们往往是因为一次特定的受伤或一个遗传体质才出问题。
那书包呢?规模最大的那篇回顾——69 篇研究、超过 72,000 名孩子——的结论是:“没有令人信服的证据显示书包使用的各个面向会增加背痛风险”(Yamato et al., 2018)。不是重量,也不是怎么背。而那还只是针对一般的背痛;至于“书包造成椎间盘突出”这个念头,证据更是薄弱。唯一一个站得住半只脚的生活方式因素是长时间坐着(一篇单一的回顾性研究点出每天坐超过六小时),但就连那个测的也是坐着,不是屏幕或书包,而且它跟那套自信满满的“手机正在折断孩子脊椎”的叙事,差了十万八千里(Qi et al., 2023)。
“但它正在上升啊!”——真的吗?
你会看到有人声称,青少年椎间盘突出正逐年攀升。老实说:我们找不到能佐证这说法的数据。这个说法在回顾性文章里被当成背景一再重复,背后却没有任何扎实的人群趋势研究撑着。真正改变的是,我们现在扫描得比以前随性多了,因此发现了更多。比起“大爆发”,“一直很罕见,只是检测得更好”是更公道的总结。
让人安心的结尾
如果那件罕见的事真的发生了,预后是好的。汇总的数据显示,手术后腿痛和功能障碍戏剧性地改善,短期内同节段的再手术率约 1%(Than et al., 2024)——而且关键是,保守治疗会先试,而且通常有效;手术是那条路失败之后才动用的最后手段(McAvoy et al., 2019)。我们会补上一句诚实的话:在数十年的尺度上,可能会有一小群、但值得注意的比例需要再动一次手术(Papagelopoulos et al., 1998)。不过“罕见、可治疗,而且通常不用手术就能处理”,才是这件事真正的样貌。
什么时候才真的该看医生
如果一个孩子或青少年有背痛或腿痛,并伴随麻木、刺痛或无力,或是会把他们痛醒、或跟在一次真实受伤之后的疼痛,又或者大小便控制有任何变化,请认真看待。那值得一次正经的评估——不是一篇博客的安慰,也不是一个恐慌下买的小工具。
诚实的结论
12 岁小孩动椎间盘手术是真实、有记录,而且极其罕见的——它是由基因、外伤和沉重的运动负荷所驱动,不是书包、手机或普通的驼背。以最好的证据来看,书包恐慌是一个迷思。所以,让群聊的惊恐退场吧,留意那些真正的危险征兆,也别为了预防一个姿势小工具根本预防不了的东西,去给你的孩子买一个姿势小工具。
